Edit: Okay I managed to make it work, TL;DR the issue was caused by Dice so Nice and it was fixed by checking the Display chat message immediately in Dice so Nice Module Settings. I think the issue is that Monk's Token Bar may not call the portions of the API that MIDI QOL is listening for. I had a note that they did not play well together.I can't seem to find any info on how the MIDI QOL damageonlyworkflow is applied or what I should be passing it. Take a look on the Midi-QoL settings, there's a configuration to automatically transfer effects on a failed save, that may not be selected. Try using some Midi-QoL/DAE SRD sample Item, that apply an effect, like bless, and see if they work. If they work the problem is in the items you made, try using the sample items as a base to make yours.

For midi QOL it's in the settings tab, under modules and midi QOL. Usually the setting button is on the right side of foundry in the last tab, and once that window is open you may find the settings if you scroll down a bit. One needs to enable the modules to see the settings. Hope this helps.
